You Deserve a Medal :: Osteoporosis New Zealand

Conditions of entry

Nominees must:

Be over 65 years of age
Be a resident in New Zealand
Not be previously acknowledged for contribution to community through physical activity
Set an example to family, colleagues or community through leading an active lifestyle
Have a planned physical exercise / activity regime
Be nominated by a colleague, associate, friend, association or family
Agree to have their name put forward and for any information provided to be used for promotional purposes (if a finalist)

The "ACC Thinksafe" medal will be presented to the nominee who has recently taken pro-active steps to remain healthy and injury-free, and acts as a role model by promoting this approach to others

Judging will be carried out by a panel consisting of members of the Board of Osteoporosis New Zealand Inc and sponsors

The panel's decision is final and no correspondence will be entered into

The finalists will be required to travel to Wellington. Reasonable travel costs will be covered

Applications close 21 September

Four awards will be made annually
